On the campus of Lakeview Hospital, we are located at 520 East Medical Drive, Suite 100, Bountiful, UT. Simply exit I-15 at 500 South in Bountiful. Go East until you reach Medical Drive and turn left. Our clinic is on the first floor of the 520 East Building, which is directly West of Lakeview Hospital.
